Request for Services

Welch-Schmidt Center for Communication Disorders

Please use this form to request speech-language therapy or evaluation services from the Welch- Schmidt Center for Communication Disorders, either for yourself or on behalf of an individual. Once submitted, the request will be placed on the waitlist and we will reach out if/when a spot opens up. 

We are a University training program, so our services are offered on a semester-basis. Scheduling for therapy services and diagnostic services takes place at the beginning of each semester, roughly early January, early June, and early September.
